Luxury Package: 750$

Luxury package includes the Healthy Kid Package and up to 10 additional visits in home throughout the calendar year. 

Healthy Kid Package: 195$  

• Yearly Holistic Wellness Exam (in home)

  • Nutrition education
  • 1 year of Doc in your Pocket
  • Developmental Assessment/Screening
  • Physical exam
  • Sports physical/Day Care form completion
  • Vision and hearing screening for children 4 and over

• One additional In Home Visit Included

  Developmental screening, Strep Ag testing, urine analysis are included in this fee.  Additional labs are priced at lab cost.

A La Cart Wellness Exam (see above): 140$

A La Cart Sick Visit 100$ (for established clients only)

 

Though I do not file directly with insurance companies you may submit the provided “superbill” to your insurance company for reimbursement as an out of network visit.  Visits may be reimbursed by an Employer Health FSA or HSA.  My goal is to be inclusive to anyone interested!
